El-Sadr, who founded the Center for Infectious Disease Epidemiologic Research (CIDER), also heads the Division of Infectious Diseases at Harlem Hospital, where she was instrumental in the development of an internationally recognized HIV/AIDS program that responded to the needs of patients from the Harlem community. Her work has focused on mother-to-child transmission of HIV and HIV/AIDS care in resource-limited setting around the world.
